To fill out the ONLINE complaint form, you need to log in or create a new account here.

If you are logged in, you can find the form in the USER ACCOUNT section, in the 3rd box, click on WITHDRAWALS, then you need to click on the button ADD NEW WITHDRAWAL, where after filling in the data, click on "SUBMIT WITHDRAWAL FORM" and you are done. We will then contact you about handling the claim.
